Registering with the surgery

The easiest way to register is to fill in the quick online form below to start the process. You do not need proof of address or immigration status, ID or an NHS number while online registration (might ask by surgery for verification)

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

To register with the surgery :

Non-urgent advice: New Patient Health Check 

On completion of your registration, you will be invited to attend a New Patient Health Check. This is a very important step in providing you the best health care we can so please try your best to attend this new patient health check appointment when invited. 

Once registered with the practice you can register for online services which will allow you to:

  • Book appointments
  • Order repeat prescriptions
  • Have access to test results

Repeat Prescription Requests

You can now view and request your repeat prescriptions online. You will be able to request a repeat prescription 7 days before they are next due.Order repeat prescriptions online

Medical Record Summary 

View a summary of your medical record information and detailed coded record (DCR) online. Accessing your DCR is subject to application and terms. View medical record information online

Identification 

After you register online or manually fill out a form, please bring the following list of documents with you to reception. 

  • For Adults:  Photo ID: Passport or Drivers Licence
  • For Under 16s: Photo ID: Passport OR Birth Certificate

Parental Access for children under 16

A receptionist will create a SystmOnline account for the child in the same way as she would for an adult, using the child’s patient record but the parent or carer would be given the child’s SystmOnline username and password. When the child reaches 16 years of age, the childs SystmOnline account becomes inactive. The parents can no longer access the account and the child must re-register to access online services themselves.

A receptionist will activate your Online Account once you have provided the necessary documents. They will provide you with a printout or send text message of your Username and Password and a Patient Information Sheet, which will give you brief instructions on how to use the online services.

Non-urgent advice: Accessing someone else’s information

As a parent, family member or carer, you may be able to access services on behalf of someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To request proxy access, please collect a proxy access form from reception between 8 am to 6:30 pm.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Non-urgent advice: Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.

Non-urgent advice: Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see someone you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
