Practice Team Development

Since the NHS has been created, the population has continued to grow and although people are beginning to live longer, there is a greater increase of chronic conditions such as diabetes and heart disease, and those people who suffer with such illnesses may need to have access to their local health services. In order to meet these needs locally, GP practices are working together with other practices and other services such as pharmacies, mental health and social care services, this is known as primary care networks (PCNs). Our practice is integrated into a Primary Care Network and therefore our practice team has also developed, we now have a Physician Associate, Clinical Pharmacists, a Pharmacy Technician, a Social Prescriber and a Physiotherapist.
